Migraine Headache during Sexual Intercourse.

 

Question: For past three weeks when I have sex with my girlfriend I’m getting extreme, throbbing pain on the left  side of my neck to back of my head. It comes fast just before I am ejaculate. It is so painful, because of that i have to stop and lay down and not able to finish the Intercourse. This is really strange and has never happened to me before, I’m worried about it.



Answer: Hi no worries, this problem may be Migraine during Sexual intercourse. This is due to At the moment of orgasm there can be a sudden, severe pain in the neck and head. This is probably due to contraction of some of the small blood vessels in the brain, similar to migraine, and in fact half the people with this type of headache are also migraine sufferers. This pain generally lasts less than an hour, but may be gone in 10 minutes or longer for a few hours. It may be throbbing, dull or stabbing. Less commonly, as sexual excitement increases, some people experience a dull, cramping, tight feeling at the back of the head. Some people have only one of these headaches, but many people have a combination, so they experience a headache that increases with sexual excitement, and culminates in an explosive headache at orgasm. Sex during pregnancy

As long as your pregnancy is proceeding normally, you can have sex as often as you like. Its better to avoid it in first 3 months and last 3 months. There is an increase in vaginal lubrication; engorgement of the genital area helps some people become orgasmic for the first time or multi-orgasmic. Unexplained vaginal bleeding is an absolute contraindications for sex during pregnancy.

 

Is it ok to have sex during pregnancy?

As long as your pregnancy is proceeding normally, you can have sex as often as you like.

But you may not always want to.

Ø At first trimister, hormonal fluctuations, fatigue and nausea may sap your sexual desire

Ø During the second trimester, increased blood flow to your sexual organs and breasts may rekindle your desire for sex

Ø During the third trimester, weight gain, back pain and other symptoms may once again dampen your enthusiasm for sex

 

But there are many reasons why sex during pregnancy can be more enjoyable, even if you are doing it less.

Can sex during pregnancy cause a miscarriage?

Most of the early miscarriages are usually related to chromosomal abnormalities or other problems in the developing baby.

Safe sexual positions in pregnancy

·        Woman on top- this allows you to control the depth of penetration, and the majority of the movement

·        Spooning- usually its best if the man is behind allowing his penis to go between your thighs and enter you from behind

·        Hands & Knees- this is a very good position for pregnant women again because of the lack of direct pressure on the abdomen

 

What about oral and anal sex?

·        Oral sex is safe during pregnancy. There’s a caveat, however

·        If you receive oral sex, make sure your partner doesn’t blow air into your vagina

·        Rarely, a burst of air may block a blood vessel (air embolism) — which could be a life-threatening condition for you and the baby

Generally, anal sex isn’t recommended during pregnancy.

Anal sex may be uncomfortable if you have pregnancy-related haemorrhoids.

More concerning, anal sex may allow infection-causing bacteria to spread from the rectum to the vagina.

 

Should my partner use a condom?

Exposure to sexually transmitted diseases during pregnancy increases the risk of infections that can affect your pregnancy and your baby’s health.

Use a condom if your partner has a sexually transmitted disease, you’re not in a mutually monogamous relationship or you choose to have sex with a new partner during pregnancy.

 

Can orgasms trigger premature labour?

Orgasms can cause uterine contractions.

But these contractions are different from the contractions you’ll feel during labor.

If you have a normal pregnancy, orgasms — with or without intercourse — don’t seem to increase the risk of premature labor or premature birth.

Bleeding during sex

Sexual Myths and Facts

Myth #1: Simultaneous orgasm is a must for a good sex life.
Fact: Most couples do not achieve simultaneous orgasm. If you are otherwise happy with your sex life there is no cause for concern.


Myth #2: Females also ejaculate when they have an orgasm.
Fact: Not true. Female orgasm does not lead to ejaculation.


Myth #3: The stock of semen in the body is limited. If you waste it by masturbating, you will soon run out of it
Fact: No, this is not true because semen is not stocked but produced in the body throughout a man’s life. It will continue be produced as long as the reproductive organs are healthy.


Myth #4: Men need or want sex more often than women.
Fact: Not true. Sexual drive varies from person to person in both men and women.


Myth #5: Only homosexuals have anal sex.
Fact: Some heterosexual couples also have anal sex.


Myth #6: Oral sex is a perversion.
Fact: No it is not a perversion. Oral sex is a matter of choice. If your partner is willing there is no harm in having oral sex.


Myth #7: Vasectomy makes a man impotent.
Fact: Vasectomy makes a man sterile, which means his semen does not contain sperms any more, but this does not mean that he is impotent. He can achieve an erection and have sex as he did earlier. Vasectomy may actually improve his sex life because there is no fear of unwanted pregnancy.

Myth #9: Losing one drop of semen is equal to losing about 40 drops of blood.
Fact:  Whoever told you this one was probably pulling your leg.  Semen is not made of blood.  Losing semen is in no way related to losing blood.


Myth #10: Having sex with a virgin will cure a man of his venereal disease.
Fact:  This is a dangerous misconception. If a man with a venereal disease has sex with a virgin, he will only end up passing it on to her. Effective medicines are available for the treatment of venereal diseases. There is no need to try such baseless and harmful methods.


Myth #11: Masturbation leads to physical & psychological problems.

Puberty – Teenage Changes for Boys and Girls

Lots of girls and guys worry about when their bodies will develop. Guys wonder when their voices will get deeper, when they’ll need to shave, or when their penises will grow. Girls want to know when their breasts will develop or when they’ll get their first period. If a friend or a younger brother or sister develops first, they may worry that there’s something wrong with them.

Different Variations of Normal
Ø  The fact is that physical development starts at different times and progresses at different rates in different people. So, the beginning of the development that comes with puberty varies from person to person — and that’s completely normal.
Ø  The earliest physical change of puberty for girls is usually breast development, which most often begins around 10 or 11 years. But it’s perfectly normal for breast development to start anytime between the ages of 8 and 13. A girl’s first menstrual period usually occurs about 2 years after breast development begins.
Ø  Boys begin their development on average around age 11 or 12, but it’s also normal to begin anytime between the ages of 9 and 15. Girls and guys who start developing earlier or later than these ranges can still be normal, but they should be checked by their doctor just to be sure.
Ø  For both boys and girls, it usually takes several years after the first changes of puberty begin before they’re all complete — and there’s a lot of variation from person to person. So, two normally developing guys or girls who are the same age can appear quite different from each other. One can look older and more physically mature than the other. But the one who started later will catch up in time.


Can I Do Anything to Speed Up Development?
ü  You can’t do anything to make your body develop faster. Of course, you should eat a nutritious diet, exercise, and get enough sleep. But special diets, dietary supplements, or creams won’t do anything to make normal puberty happen quicker.
ü  But some unhealthy things can interfere with development. Steroids, like those used for bodybuilding, can cause problems with growth, shrink testicles (and lead to breast development) in guys and shrink breasts, disrupt menstrual cycles, and cause excessive hair growth in girls. (Steroids can cause other serious health problems, too.) Girls who have eating disorders or who exercise compulsively may delay their development and hurt their health. A number of medical conditions can also delay puberty and development.
ü  If you’re a girl who’s 13 or older or a boy who’s 15 or older and you haven’t shown any signs of puberty (or if you’re younger but are still worried), talk with your doctor. Most of the time everything is just fine, but the doctor can determine if there’s a problem. And if there is a problem, there’s almost always treatment available that will help you develop.

1. What counts as losing your virginity?
This is a question that is hard to answer because different people have many different opinions on this; some people even say that there are different types of virginity. Most people agree with the answer that if you have had penetrative sex, then you are not a virgin. Others agree that if you have experienced an orgasm than you are not a virgin.


2. Can oral sex be dangerous for a woman?
There is only a slight chance that a gas bubble in the bloodstream can happen, this is called embolism, and it is very, very rare. This could happen if the guy were to blow EXTREMELY hard into your vagina. But since oral sex is supposed to be gentle to create a pleasurable sensation, I wouldn’t worry.


3. If the girl doesn’t have an orgasm can she still get pregnant?
Yes, a female’s orgasm is not a necessary part of becoming pregnant and there is enough sperm in the pre-ejaculatory and ejaculatory fluid of males to cause a pregnancy.


4. Is it true that you can’t get pregnant if you have sex in water?
No, it’s not true. Water does not wash away sperm and is not a good birth control method.


5. Are two condoms better than one for protection from pregnancy or STDs?
No, two condoms offer LESS protection as the friction between them makes breakage more likely. The only thing that I would suggest using more than one of is VCF’s because it is a spermicidal film that is inserted into the vagina and dissolves to trap and kill sperm.


6. Can you pass on or become infected with STDs as a result of kissing?
No, you cannot pass on or become infected with an STD as a result of kissing. However, you could pass on cold sores (herpes simplex, HSV1) to your partner through kissing.


7. What are the STDs out there that I should be aware of?
Genital Herpes appear as one or more blisters on or around the genitals or rectum. Although the infection can stay in the body indefinitely, the number of outbreaks tends to decrease over a period of years. Genital Herpes occur in one out of four women and in men one out of five. Genital Herpes have been found to be released from sores or fever blisters on the mouth and lips. Oral to genital or genital to genital contact can spread the infection. Chlamydia is another sexually transmitted disease that is caused by bacteria. Most people that are infected with Chlamydia do not have any symptoms are not aware of their infections. Chlamydia can be easily treated and cured. Gonorrhea is another STD cause by a bacterium that can grow and multiply easily in warm, moist areas of the reproductive tract, as well as the mouth, throat, eyes, and anus.
Gonorrheal infections occur in 125 of 100,000 people. HIV is a retrovirus that causes AIDS, a condition in which the immune system begins to fail. HIV can be transferred by blood, semen, vaginal fluid, or breast milk. There is currently no cure for HIV or AIDS.


8. Which STDs are transmitted through oral sex?
Hepatitis A, B and C Herpes, especially if the sores are in the infectious stage, HIV (Human Immunodeficiency Virus) is fairly low risk in terms of transmission via oral sex, Gonorrhea, Syphilis, NSU (Non-specific urethritis), Chlamydia, Yeast infections such as thrush are fairly low risk in terms transmission via oral sex, and Genital Warts are all transmitted through unprotected oral sex.


9. Am I ready for sex?
Here are some questions you want to ask yourself before going all the way. Are you doing this because you want to or are you doing it because of peer pressure or maybe your partner is saying things like “everyone else is doing it” or “you would do it if you loved me”. These reasons are not reasons for you to have sex. You should do it because YOU want to. Not because your partner or friends want you to. Do I know enough about sex? Do you know how and why a woman can get pregnant? Do you know your partner well enough? You need to be able to trust them and be comfortable with them, and it will definitely be a lot better with someone that you love. Is it easy to talk to your partner about sex? Honesty is the best policy as well as being with someone that is easy to talk to about important things, such as sex. Do you know how to be safe? Do you know or have chosen your contraception? Consider these things before having sex.


10. Does masturbating several times a day cause infertility?
Masturbation has no known medical ill effects. Masturbation does not cause infertility.


11. Where is the G-spot and what is it?
The G-spot is an area located behind the front wall of the vagina, between the back of the pubic bone and the cervix. When stimulated, it can produce intensely pleasurable feelings for the woman, and may cause her to have an orgasm.


12. Where is the clitoris and what is it?
The clitoris is a small pea-sized object on the exterior of the vagina just above the vulva. It is very sensitive and when stimulated can lead to orgasm. 


13. Is there a way of stopping premature ejaculation?
The best way that some men have found to stop premature ejaculation is to think about something dull to regain some control.


14. Is there a male contraceptive pill?
There are no male contraceptive pills to take at the moment however; some companies are trying to develop a pill for  men.


15. Is there a time to have sexual intercourse when it is safe and the woman will not get pregnant?
There is no safe time to have unprotected sex if you wish to avoid pregnancy; even unprotected sex during a woman’s period can result in pregnancy.


16. What should you do if a condom breaks?
If a condom breaks while having sex and before ejaculation, stop and pull out and apply a new condom. However if ejaculation has occurred, shower or wash your genital areas with soap and water thoroughly. Never douche after a condom breaks this can force infectious bacteria deeper into the vagina. Go to a women’s hospital or gynecologist to get tested for pregnancy and sexually transmitted diseases. Amongst anything, take a deep breath and stay calm.


17. Is anal sex safe?
Yes, however if you are not relaxed it can be extremely painful because the anus is not self lubricating or elastic and the muscles will clench on contact. But it is not safe against pregnancy or STDs.


18. How long does it take for birth control pills to work?
Birth controls vary. Depending on the type of pill you use and when you start the packet of pills all depends on when the pills become effective. Check with your healthcare provider to when the particular version of the pill will become effective against pregnancy.


19. Can I stop using condoms once you start using birth control?
Birth control may protect against pregnancy but not STD’s. Condoms in use with birth control pills will give extra protection.


20. Does taking birth control pills have any side effects?
All women are different; however the most common side effects are mood swings, weight gain, breast tenderness, nausea and headaches.


21. Can taking birth control pills give false positive pregnancy results?
The contraceptive pill would not make a pregnancy test have a positive result. Pregnancy tests look for hormones in the body that are only present when someone is pregnant. But missing taking the pill can put you at risk for pregnancy. 


22. After putting on a condom he realizes that it is on inside out and then changed it the right way is it still safe to use it?
It is risky, this is because the penis leaks a few drops of semen as soon as it is hard and this could have got onto the condom.


23. Why do I feel like I have to pee every time I have sex?
Anxiety related to having sex can cause the thought that you have to go to the bathroom as well as some sexual positions can add pressure on the bladder to give you the impression that you have to pee when you don’t.


24. Are tampons dangerous to use?
By using tampons you increase the risk of getting Toxic Shock Syndrome. 1 to 17 of every 100,000 menstruating women and girls will get the syndrome due to bacterium that can be found in the vagina of some women.


25. How long can sperm survive?
Sperm can live in the body for three days and can survive up to seven days.


26. Can you get pregnant by swallowing semen?
The millions of sperm that is contained in semen needs to travel up the vagina and through the cervix into the uterus to fertilize the egg. The egg can not be fertilized by semen being swallowed down the throat.


27. Is it ok to have sex during a yeast infection?
It is not suggestible to have genital to genital contact when having a yeast infection because it can be transmitted between partners.


28. Does having sex cause a change in your periods?
Being sexually active does not change your periods, however stress, nutrition, exercise changes, medications and infections can.


29. Is having sex daily normal?
It is perfectly normal and part of human nature.


30. Is pulling out safe?

Question: What is meant by “healthy” sexual relationship?
Answer: A healthy sexual relationship is different for every couple because every individual has differing sexual needs. While the activities involved in each sexual relationship can vary widely, in general, “healthy” sex should encompass the following:
ü  Both partners should feel equally pleased with the activities.
ü  Neither partner should feel forced into doing something they don’t want to do.
ü  The right to say “no” to sex at any time, for any reason.
ü  Mutual respect before and after sex.
ü  Neither party suffers a loss of self-esteem.
ü  Trust exists as well as openness about sexual history and current activities.




Question: Is there anything you can do to change or increase your sex drive?
Answer: Experts say the answer lies not so much in what your sex drive is, but whether or not it has reached its potential. While everyone’s level of desire is individual, it can also differ with each partner, and vary within a relationship, increasing or decreasing over time.

What is sexually transmitted disease?
Sexually transmitted diseases (STDs) are diseases that can spread from one person to another by sexual contact. STDs can cause pain, and some can cause infertility and even death if not treated. Some common curable STDs are gonorrhea, trichomoniasis, chlamydial infection, syphilis and HIV/AIDS.
Each year there are more than 333 million new cases of curable STDs, one million new cases of HIV infection, and millions of other viral STDs such as herpes and hepatitis B.

How sexually transmitted diseases be affected?
By definition STD’s are diseases contracted during sexual contact. It isimportant to keep in mind that sexual contacts, as far as STDs are concerned, includes more than just sexual intercourse (vaginal and anal). Kissing, oral-genital contact, and the use of sexual “toys,” such as vibrators too can cause STDs.


How to prevent STDs?
People can avoid STDs by changing their sexual behaviour. They can follow any of the ABCs:
A.Abstain from sex -This is the only guaranteed protection.
B.Be mutually faithful -Always have sex with the same person. This person must not have sex with any other person and must not have a STD.
Important: You cannot usually tell whether a person has a STD just by looking at him or her. People with STDs, including HIV, usually do not look sick.
C.Consistently use condoms -Use them every time and use them correctly. To prevent STDs, people at risk should use condoms even when they use another family planning method. If a woman’s sex partner is not willing to use condoms, she should try to use spermicide. It is important to note that spermicides do not stop HIV/AIDS. The diaphragm and cervical cap may also help prevent some STDs.


All STD’s treatable?
Most STDs are treatable. However, even the once easily cured gonorrhea has become resistant to many of the older traditional antibiotics. Other STDs, such as herpes, AIDS, and genital warts, all of which are caused by viruses, have no cure. Some of these infections are very uncomfortable, while others can be deadly.


Getting Treated
Many STDs can be treated and cured, especially in their early stages. Some, such as HIV and herpes, cannot be cured, but sometimes their effects can be stopped for a time.
Prevention is better than cure


Sometimes scarring or infertility can follow.
A person who thinks he or she may have STD should:
ü  Get diagnosed and treated immediately.
ü  Take all of the medicine according to instructions, even if the symptoms go away. The medicines can cause some side effects, such as vomiting, diarrohea, or rash. If any of these side effects occurs and is severe, the person must return to the clinic that provided the medicine. All of the medicine must be taken for a lasting cure.
ü  Avoid sex with anyone until three days after the treatment is finished and all symptoms are gone.
ü  Tell his or her sex partner or partners so that they get treated too. Unless all sex partners are treated at the same time, they may infect each other again and again. It is especially important for a man to tell a woman.
This is because many women do not have symptoms until the STD has reached a more serious stage.


If friends have symptoms, urge them to seek care. Urge them to use condoms and/ or spermicide and to see a health care provider for a check up. There really is no such thing as “safe” sex. The only truly safe sex is abstinence. Sex in the context of a monogamous 
relationship wherein neither party is infected with a STD is also considered “safe”. Most people think that kissing is a safe activity. Unfortunately, syphilis, herpes, and other diseases can be contracted through this relatively simple and apparently harmless act. All other forms of sexual contact carry some risk. Condoms are commonly thought to protect against STDs. Condoms are useful in preventing certain diseases, such as herpes and gonorrhea. However, they do not fully protect against other diseases such as genital warts, syphilis and AIDS.

Is masturbation harmful?

What’s the average size of the penis?
The average size of penis is 15cm, the normal range is considered to be between 13 and 18cm.


Is penis size important?
The fact of the matter is that it is important only if you think so. Factually speaking, the vagina is only eight to thirteen centimetres long, and only the outer one third of it is supplied with nerves and is therefore sensitive to touch. Therefore even a very small penis can touch every square centimeter within the vagina and can certainly perform adequately.


Its is possible to increase penis size?
Yes. There are two surgical procedures to increase penis size– the Bihari Procedure, and Fat Injection.
The Bihari Procedure consists of cutting the ligament that secures the base of the penis to the body. This gives between one-half and two inches of increased length to the penis; however, because the penis is no longer anchored to the body an erection no longer points ‘up.’
Fat Injection is the process of removing fat from the backs of the thighs and injecting it into the body of the penis to make the penis thicker. Because the body rejects a significant portion of the injection this procedure may need to be repeated several times and each operation carries with it a severe risk of infection.

My penis bends down. What is wrong with it?
One-quarter of all penises bend in some direction and some bend downward even when erect. Coming to think of it, one would really thank that the penis doesn’t have any bones in it, and therefore it can normally easily bend in many directions to perform its job without much of discomfort. Unless the bend is severe or causes you pain, there is nothing wrong or abnormal about your penis. It should not interfere with sexual intercourse. In fact some people find the bend is to their advantage, making penetration easier.


One of my testicles is larger than the other. Is it normal?
Rarely are both the testicles identical. In fact the left one hangs lower in 85% of cases. It is nothing to worry about.


Sometimes I feel pain in my testicles. Any problem with that?
Intermittent twinges in the testicles are common and these sometimes happen after the intercourse. If it lasts less than a minute then there is nothing to worry. An infection or inflammation causes testicular pain that builds up gradually. In such cases consult your doctor.


Am ejaculating too fast, What do I do?

Is Erections are normal for teen age boys?
Yes. Erections are a perfectly normal function of the male body, especially in guys who are going through puberty.


What Is mean by Erection?
Ø  An erection is a hardening of the penis that occurs when sponge-like tissue inside the penis fills up with blood. Usually, an erection causes the penis to enlarge and stand
Ø  Erections can go away on their own or after ejaculation, the release of semen through the urethra, the small hole at the tip of the penis.

Know about Infertility
Infertility implies the failure to conceive. It is termed
v  Primary: when conception has never occurred and
v  Secondary: when the patient fails to conceive after having a child.


How conception happens.
Conception results from the fertilisation of the ovum (female gamete) by the spermatozoa or sperm (male gamete). After ejaculation the sperms migrate towards the fallopian tubes and fertilisation takes place in the fallopian tubes. The usual life span of a sperm is estimated to be about 24 hours; while the life span of ovum is estimated at 12 hours. So a coitus to be capable of fertilisation, must
take place within twelve hours of ovulation (release of ovum from the ovary).
That certainly explains why some married couples fail to conceive, though they are potentially fertile. The intercourse should be able to coincide with the short time after ovulation, when the ovum is viable, so that chances of fertilisation are there. And even then there are numerous factors like sperm motility, viability of sperms, etc. before fertilisation can eventually occur.


When does ovulation occur?
It has been concluded after considerable research that ovulation most commonly occurs fourteen days before the onset of the next menstrual period. For example, ovulation will occur on the 14th or 15th day in a 28 days cycle, and on the 10th day in a 24 days cycle. 


Female’s fault if conception is not occurring – Is it true?
This is a misconception because conception depends upon the fertility of both the partners. Any fault in either partner can lead to inferetility. Studies have shown that 40% of infertility is due to the male partner.


Main causes of male infertility
The commonest reason, is a low sperm count, which can be attributed to various causes;
Ø  Idiopathic i.e., when the exact cause is not known.
Ø  Varicocele – It is a swollen varicose vein in the scrotum, most commonly on the left side. Blood accumulated in the swollen testicular veins leads to increased temperature in the testis & damages sperm production.


It can be diagnosed, by asking the patient to stand erect and cough . The cord like structure from which the testis hangs i.e., the spermatic cord, feels like a “bunch of worms”, on palpation.
  • Duct blockage – Infection like gonorrohea, TB, filariasis; on surgery for hernia/hydrocele may lead to blockage of the passage between the testes and the penis. In this case sperms will be absent from the
    semen(azoospermia). 
  • Hormonal imbalance – Hormonal imbalance due to pituitary or adrenal gland can lead to infertility.
  • Substance Abuse – Exessive intake of alcohol and/or drugs leads to decrease in liver function and increase in levels of the
    hormone Estrogen which in turn suppresses sperm production.
  • Testicular factors – like twisting of testis leading to compromised blood supply; undescended testes when the testes lie in the abdomen; and damage due to exercise or heat can decrease sperm production. Testes lie outside the body in the scortum because they can produce sperms in a cooler environment. So clothes like tight jeans, nylon briefs, and hot baths/saunas, lead to damage to sperm production.


Effect of long term medication- certain drugs have an adverse effect on the sperm production. Some of these are  antihypertensives like reserpine, methyldopa, guanethidine & propranolol
ü  Corticosteroids
ü  Anabolic steroids
ü  Antipsychotics
ü  Certain anti-cancer drugs


People working with harmful chemicals like lead, nickel, mercury, anaesthetic gases, insecticides & pesticides and x-rays, are at increased risk of suffering from decreased sperm count.
Problems with ejaculation- Certain men have problems with ejaculation like problem having an erection (impotence) or inadequate erection for vaginal penetration.


Problems with erection can be due to psychological factors like performance anxiety, inhibitions against sex and due to physical factors like neurologic defects, diabetes, inadequate blood flow to the penis, and also to certain hormonal problems.


Certain tests like monitoring for nocturnal erections and measuring the blood flow through the penis may need to be carried out.


A successful alternative that has come up is collections of sperms by masturbation which later be used for artificial insemination.


Another problem with ejaculation that some men face is that the semen goes backwards into the urinary bladder instead of being ejaculated through the penis. This can occur in men who have had spinal injury or the ones suffering from diabetes, causing improper contraction of urinary bladder sphincter muscle.


Infections – another common reason for male infertility are infections like tuberculosis, chlamydia, gonorrhoea and syphilis. Mumps can lead to complications like testicular inflammation. Infections can damage the male reproductive tract leading to duct obstruction in some cases and decreased sperm production in others.


These have to be treated by antibiotics and semen culture can tell whether infection is controlled or not.


How to promote fertility?
ü  Smoking & excessive alcohol intake is associated with low sperm counts. These habits should be discontinued.
ü  Stop using drugs such as body-building steroids & illicit drugs. Other medication such as antihypertensives should also be reviewed in the light of effect on sperm count.
ü  Most importantly the frequency of intercourse should be increased, so that chances of conception increase.
ü  Men should wear loose fitting underwear and cool clothes and avoid high temperature baths like saunas etc.
ü  It is advisable that when the couple is planning pregnancy, intercourse is done in a proper position and using proper technique. Lubricants such petroleum jelly etc. should be avoided.


Investigations help to find fertility
A simple test is Semen Analysis.
The pre-requisites are –
  • Abstinence from sex or masturbation for 4-5 days prior to the day of test.
  • The sample given should not be more than half hour old
  • It should be collected in a wide-mouthed, clean jar by masturbation.
  • The sample is to be kept at room temperature.
Other sophisticated tests like sperm viability test, sperm penetration assay, antisperm antibodies test may be required in certain cases.
Testicular function can be assessed by male hormone levels in blood of the male, or by testicular biopsy.

How to Prevent from Testicular Injuries
It’s a good idea to take precautions to avoid testicular injuries, especially if you play sports, exercise a lot, or just live an all-around active life. Here are some tips to keep your testicles safe and sound:
  • Protect your testicles. Always wear an athletic cup or athletic supporter when playing sports or participating in strenuous
    activity. Athletic cups are usually made of hard plastic, are worn over the groin area, and provide a good degree of shielding and safety for the testicles. Cups are best used when participating in sports where your testicles might get hit or kicked, like football, hockey, soccer, or karate. An athletic supporter, or jock strap, is basically a cloth pouch that you wear to keep your testicles close to your body. Athletic supporters are best used when participating in strenuous exercise, cycling, or doing any heavy lifting.
  • Check your fit. Make sure the athletic cup and/or athletic supporter is the right size. Safety equipment that’s too small or
    too big won’t protect you as effectively.
  • Keep your doctor informed. If you play sports, you probably have regular physical exams by a doctor. If you experience testicular pain even occasionally, talk to your doctor about it.
  • Be aware of the risks of your sport or activity. If you play a sport or participate in an activity with a high risk of injury, talk to your coach or doctor about any additional protective gear you should use.
Participating in sports and living an active life are great ways to stay fit and relieve stress. But it’s important to make sure your testicles are protected. When you’re exercising or playing sports, make sure that using protective gear is part of your routine and you’ll be able to play hard without fear of testicular injury!

Doctor Answer:  Hi no worries, You are reaching your adulthood, Chances are that you had a “wet dream” — something that can be embarrassing and confusing to teen boys, but is completely normal.

A wet dream is also known as a nocturnal emission. Nocturnal means “at night” and emission means “discharge.” This makes sense because a wet dream is when semen – the fluid containing sperm is discharged from the penis during ejaculation while a boy’s asleep. Usually wet dreams occur during dreams that have sexual images. Sometimes boys wake up from a wet dream, but sometimes they sleep through it.

Wet dreams begin during puberty when the body starts making more testosterone, the major male hormone. Causes for Delayed Puberty

Puberty can be delayed for several reasons. Most often, it’s simply a pattern of growth and development in a family. A guy or girl may find that his or her parent, uncle, aunt, brothers, sisters, or cousins developed later than usual, too. This is called constitutional delay (or being a late bloomer), and it usually doesn’t require any kind of treatment. These teens will eventually develop normally, just later than most of their peers.
Medical problems also can cause delays in puberty. Some people with chronic illnesses like diabetes, cystic fibrosis, kidney disease, or even asthma may go through puberty at an older age because their illnesses can make it harder for their bodies to grow and develop. Proper treatment and better control of many of these conditions can help make delayed puberty less likely to occur.


A person who’s malnourished — without enough food to eat or without the proper nutrients — may also develop later than peers who eat a healthy, balanced diet. For example, teens with the eating disorder anorexia nervosa often lose so much
weight that their bodies can’t develop properly. Girls who are extremely active
in sports may be late developers because their level of exercise keeps them so
lean. Girls’ bodies require a certain amount of fat before they can go through
puberty or get their periods.


Delayed puberty can also happen because of problems in the pituitary or thyroid glands. These glands produce hormones important for body growth and development.
Some people who don’t go through puberty at the normal time have problems with their chromosomes, which are made up of DNA that contain our body’s construction plans. Problems with the chromosomes can interfere with normal growth processes.
Turner syndrome is an example of a chromosome disorder. It happens when one of a female’s two X chromosomes is abnormal or missing. This causes problems with how a girl grows and with the development of her ovaries and production of sex hormones. Women who have untreated Turner syndrome are shorter than normal, are usually infertile, and may have other medical problems.
Males with Klinefelter syndrome are born with an extra X chromosome (XXY instead of XY). This condition can low sexual development.

 

Role of Doctors

The good news is that if there is a problem, doctors usually can help teens with delayed puberty to develop more normally. So if you are worried that you’re not developing as you should, you should ask your parents to make an appointment with your doctor.
In addition to doing a physical examination, the doctor will take your medical history by asking you about any concerns and symptoms you have, your past health, your family’s health, any medications you’re taking, any allergies you may have, and other issues like growth patterns of your family members. He or she will chart your growth to see if your growth pattern points to a problem and also may order blood tests to check for thyroid, pituitary, chromosomal, or other problems. You may also have a “bone age” X-ray, which allows the doctor to see whether your bones are maturing normally.
Some teens who are late developers may have a difficult time waiting for the changes of puberty to finally get going — even after a doctor has reassured them that they are normal. In some cases, doctors may offer teens a short course (usually a few months) of treatment with hormone medications to get the changes of puberty started. Usually, when the treatment is stopped a few months later, the teen’s own hormones will take over from there to complete the process of puberty.

 

How to Deal With Delayed Puberty

It can be really hard to watch your friends grow and develop when the same thing’s not happening to you. You may feel like you’re never going to catch up. People at school may joke about your small size or your flat chest. Even when the doctor or your parents reassure you that things will be OK eventually — and even when you believe they’re right — it’s difficult to wait for something that can affect how you feel about yourself. 
Delayed puberty can be difficult for anyone to accept and deal with — but it’s a problem that usually gets solved. Ask for help if you have any concerns about your development. And remember that in most cases kids will eventually catch up with their peers.
If you’re feeling depressed or having school or other problems related to delays in your growth and development, talk to your mom or dad, your doctor, or another trusted adult about finding a counselor or therapist you can talk to. Can I have Sex during periods?

Many peoples asking the above questions often, here few common questions frequently asked by peoples

Many men and women feel anxious about having sex during periods because we have been culturally conditioned to believe that menstruation is something dirty. There are also many misconceptions about the medical aspects of sex during periods. Medically, it is normal to have sex during periods. The safe sex practices required to avoid pregnancy and STDs when a woman is not menstruating should also be used during periods.

 

Can sex during periods result in pregnancy? 

Chances of pregnancy during periods are minimal. However, if you strictly want to avoid pregnancy it is advisable to use an adequate birth-control method each time you have intercourse, including during periods. Though the chances of pregnancy during periods are very low, they are not entirely absent because the sperm can survive in the body for a few days and there is a small chance that an early ovulation will cause pregnancy.

 

Will contact with menstrual blood cause sexually-transmitted diseases (STD)?

Menstrual blood is nothing but normal human blood mixed with tissues lining the uterus. Contact with menstrual blood is not harmful in any way. If your partner has a STD, you have a high chance of contracting it and you should be using a condom during intercourse, both during periods and otherwise. 
 

Does sex during periods cause HIV infection?

Sex during periods cannot cause HIV infection unless one of the partners is HIV infected. Unprotected intercourse with an HIV infected partner can cause HIV infection, regardless of periods. The only connection between HIV and sex during periods is that the chances of HIV transmission from an HIV infected person to his/ her partner are higher if they have sex during periods. If you have the slightest concern about HIV infection, the thumb rule is to use a condom regardless of periods.

 

Will contact with menstrual blood harm the penis?

No, menstrual blood will not harm the penis in any way. The worries and concerns about menstrual blood stem from the fact that we have been socially conditioned to see it as a dirty fluid. Scientifically, menstrual fluid is a mix of blood and tissues that line the uterus every month to prepare it for pregnancy. If pregnancy does not occur, the tissue is shed because it is no longer required.

 

Can sex during periods injure the uterus?

No, sex during periods cannot injure the uterus. There is a common perception that the mouth of the uterus opens up during periods and the penis can poke into it and hurt the uterus. This is not correct. Menstrual flood oozes out of a very small opening in the mouth of the uterus. The penis can never poke in through this opening. 

 

Is sex during periods a perversion?

You would be happy to know that a large number of couples have sex during periods. From the medical standpoint, sex during periods is absolutely normal. So if you too indulge in it, there is absolutely no need to feel guilty or anxious.

 

Will sex during periods stop bleeding?

Some women do notice that their periods stop within a day or so of sexual intercourse. This does not mean that the menstrual blood has been pushed back into the uterus and is unable to flow out.  It happens primarily because sexual intercourse causes uterine contractions that expel the menstrual fluids and tissue faster, thus causing the bleeding to stop sooner than usual.

Tonsillitis Symptoms :
1. Sore throat: Pain in the throat is the most common presentation. However, young babies may not present with the pain but may present with an inability to eat.
2. Dysphagia: That is difficult swallowing. This may either be due to pain or due to huge increase in the size of the tonsils due to frequent inflammation.
3. Fever: Acute infection of the tonsils may present with moderate to high rise in body temperature. In case of septic foci on the tonsils, there may be fever with chills.
4. Apnoea: Sleep apnoea, a disturbed sleep disorder may occur due to an obstruction to the
air passage due to heavily enlarged tonsils or adenoids. However, it is interesting to note that most children with sleep apnoea do not recover of this disorder after the removal of the tonsils. (This is because, in many cases, there are associated causes for apnoea, such as obesity, adenoids, small jaw, bony skull deformity (congenital), neuromuscular disorder, etc.)
5. Snoring: Due to obstructed air passage caused by the enlarged tonsils, snoring may be one of the symptoms in many children suffering from recurrent tonsillitis.

Hyperthyroidism
Hyperthyroidism means the thyroid gland is overactive. If you have persistent or recurrent problems with sexual response — and if these problems are making you distressed or straining your relationship with your partner — what you’re experiencing is known medically as female sexual dysfunction.
Female sexual dysfunction has many possible symptoms and causes. Fortunately, they’re almost all treatable. Communicating your concerns and understanding your anatomy and your body’s normal response to sexual activity are important steps toward gaining sexual  satisfaction.

Symptoms
You can develop female sexual dysfunction at any age, but sexual problems are most common when your hormones are in flux — for example, when you’ve just had a baby or when you’re making the transition into menopause. Sexual concerns may also occur with major illness, such as cancer.  Your problems might be classified as female sexual dysfunction if you experience one or more of the following and you’re distressed about it:
  • Lack of sexual desire -your desire to have sex is low or absent.
  • Inability to become aroused -You can’t maintain arousal during sexual activity, or you don’t become aroused despite a desire to have sex.
  • Lack of orgasm, or sexual climax -You cannot experience an orgasm.
  • Painful intercourse-You have pain during sexual contact.


Causes
Several factors may contribute to sexual dissatisfaction or dysfunction. These factors tend to be interrelated.
  • Physical. Physical conditions that may cause or contribute to sexual problems include arthritis, urinary or bowel difficulties, pelvic surgery, fatigue, headaches, other pain problems, and neurological disorders such as multiple sclerosis. Certain medications, including some antidepressants, blood pressure medications, antihistamines and chemotherapy drugs, can decrease your sex drive and your body’s ability to achieve orgasm.
  • Hormonal. Lower oestrogen levels during the menopausal transition may lead to changes in your genital tissues and your sexual responsiveness. The folds of skin that cover your genital region (labia) become thinner, exposing more of the clitoris. This increased exposure sometimes reduces the sensitivity of the clitoris, or may cause an unpleasant tingling or prickling sensation.
  • In addition, the vaginal lining becomes thinner and less elastic, particularly if you’re not sexually active. At the same time, the vagina requires more stimulation to relax and lubricate before intercourse. These factors can lead to painful intercourse (dyspareunia), and achieving orgasm may take longer. Your body’s hormone levels also shift after giving birth and during breast-feeding, which can lead to vaginal dryness and can affect your desire to have sex.
  • Psychological and social. Untreated anxiety or depression can cause or contribute to sexual dysfunction, as can long-term stress. The worries of pregnancy and demandsof being a new mother may have similar effects. Longstanding conflicts with your partner — about sex or any other aspect of your relationship — can diminish your sexual responsiveness as well. Cultural and religious issues and problems with your own body image also may contribute.
  • Emotional distress can be both a cause and a result of sexual dysfunction. Regardless of where the cycle began, you usually need to address relationship issues for treatment to be effective.

  • Strengthen pelvic muscles. Pelvic floor exercises can help with some arousal and orgasm problems. Doing Kegel exercises strengthens the muscles involved in pleasurable sexual sensations. To perform these exercises, tighten your pelvic muscles as if you’re stopping your stream of urine. Hold for a count of five, relax and repeat. Do these exercises several times a day. Your doctor also may recommend exercising with vaginal weights — a series of five weights, each increasingly heavier, that you hold in place in your vagina — to strengthen pelvic floor muscles. You gradually work up to heavier weights as your muscle tone improves.
  • Seek counselling. Talk with a counsellor or therapist specializing in sexual and relationship problems. Therapy often includes education about normal sexual response, ways to enhance intimacy with your partner, and recommendations for reading materials or couples exercises. With a therapist’s help, you may gain a better understanding of your sexual identity, beliefs and attitudes; relationship factors including intimacy and attachment; communication and coping styles; and your overall emotional health.
Sex therapy is a short-term form of counselling, generally involving 5 to 20 sessions with a sex therapist. A typical session may be one hour every week or every other week. During the session, the counsellor will give the patient “assignments” to do at home, such as:
  • Reading books about sexuality
  • Touching exercises that are designed to take away the pressure to perform during sex
  • Practicing better sexual communication skills
Sex therapy may be useful for treating erectile dysfunction if a man is able to have a normal erection during sleep, the results of his physical examination and blood tests are normal and he is generally in good health. Sex therapy may also be helpful when erectile dysfunction is caused by stress, such as: work worries, financial worries, relationship conflicts, and poor sexual communication. In these cases, sex therapy may be the best treatment option.

Sex therapy is most effective when a man’s sexual partner is willing to be part of the treatment. The kidney acts as a filter for blood, removing waste products from the body and helping regulate the levels of chemicals important for body function. The urine drains from the kidney into the bladder through a narrow tube called the ureter. When the bladder fills and there is an urge to urinate, the bladder empties through the urethra, a much wider tube than the urethra. And some people, the urine chemicals crystallize and form the beginning, or a nidus, of a kidney stone. These stones are very tiny when they form, smaller than a grain of sand, but gradually they can grow to a quarter inch or larger. The size of the stone doesn’t matter as much as where it is located.
When the stone sits in the kidney, it rarely causes problems, but should it fall into the ureter, it acts like a dam. The kidney continues to function and make urine, which backs up behind the stone, stretching the kidney. This pressure build up causes the pain of a kidney stone, but it also helps push the stone along the course of the ureter. When the stone enters the bladder, the
obstruction in the ureter is relieved and the symptoms of a kidney stone are resolved.

Kidney Stones Causes

There is no consensus as to why kidney stones form.
  • Heredity: Some people are more susceptible to forming kidney stones, and heredity certainly plays a role. The majority of kidney stones are made of calcium, and hypercalciuria (high levels of calcium in the urine), is a risk factor. The predisposition to high levels of calcium in the urine may be passed on from generation to generation. Some rare hereditary diseases also predisposecystine (an amino acid), oxalate, (a type of salt), and uric acid (as in gout).

  • Diet: Diet may or may not be an issue. If a person is susceptible to forming stones, then foods high in calcium may increase the risk, however if a person isn’t susceptible to forming stones, nothing in the diet will change that risk.
  • OTC products: People taking diuretics (or “water pills”) and those who consume excess calcium-containing antacids can increase the amount of calcium in their urine and increase their risk of forming stones. Patients with HIV who take the medication indinavir (Crixivan) can form indinavir stones.

 

Kidney Stones Symptoms

When a tubular structure is blocked in the body, pain is generated in waves as the body tries to unblock the obstruction. These waves of pain are called colic.
  • Renal colic (renal is the medical term for things related to the kidney) has a classic presentation when a kidney stone is being passed. 
  • The pain is intense and comes on suddenly.
  • It is usually located in the flank or the side of the mid back and radiates to the groin. Those affected cannot find a comfortable position, and many writhe in pain.
  • This is opposed to non-colicky type pain, like appendicitis or pancreatitis, where movement causes increased pain and affected persons hold very still.
  • Sweating, nausea and vomiting are common.
  • Blood may be visible in the urine because the stone has irritated the ureter. Blood in the urine, however, does not always mean a person has a kidney stone. There may be other reasons for the blood, including kidney and bladder infections, trauma, or tumors. Urinalysis with a microscope may detect blood even if it is not appreciated by the naked eye. Sometimes, if the stone causes complete obstruction, there may be no blood in the urine because it cannot get past the stone.

What is Polycystic Ovarian Syndrome (PCOS)?
PCOS is a health problem that can affect a woman’s menstrual cycle, fertility, hormones, insulin production, heart, blood vessels, and appearance. Women with PCOS have these characteristics:
  • High levels of male hormones, also called androgens
  • An irregular or no menstrual cycle
  • May or may not have many small cysts in their ovaries. Cysts are fluid-filled sacs.

PCOS is the most common hormonal reproductive problem in women of childbearing age.


How many women have Polycystic Ovarian Syndrome (PCOS)?
An estimated five to 10 percent of women of childbearing age have PCOS.


Causes for Polycystic Ovarian Syndrome (PCOS)
No one knows the exact cause of PCOS. Women with PCOS frequently have a mother or sister with PCOS. But there is not yet enough evidence to say there is a genetic link to this disorder. Many women with PCOS have a weight problem. So researchers are looking at the relationship between PCOS and the body’s ability to make insulin. Insulin is a hormone that regulates the change of sugar, starches, and other food into energy for the body’s use or for storage. Since some women with PCOS make too much insulin, it’s possible that the ovaries react by making too many male hormones, called androgens. This can lead to acne, excessive hair growth, weight gain, and ovulation problems.


Do women with Polycystic Ovarian Syndrome (PCOS) have trouble with their menstrual cycle?
The ovaries are two small organs, one on each side of a woman’s uterus. A woman’s ovaries have follicles, which are tiny sacs filled with liquid that hold the eggs. These sacs are also called cysts. Each month about 20 eggs start to mature, but usually only one becomes dominant. As the one egg grows, the follicle accumulates fluid in it. When that egg matures, the follicle breaks open to release the egg so it can travel through the fallopian tube for fertilization. When the single egg leaves the follicle, ovulation takes place. In women with PCOS, the ovary doesn’t make all of the hormones it needs for any of the eggs to fully mature. They may start to grow and accumulate fluid. But no one egg becomes large enough. Instead, some may remain as cysts. Since no egg matures or is released, ovulation does not occur and the hormone progesterone is not made. Without progesterone, a woman’s menstrual cycle is irregular or absent. Also, the cysts produce male hormones, which continue to prevent ovulation.


Symptoms of Polycystic Ovarian Syndrome (PCOS)
These are some of the symptoms of PCOS:
  • Infrequent menstrual periods, no menstrual periods, and/or irregular bleeding
  • Infertility or inability to get pregnant because of not ovulating
  • Increased growth of hair on the face, chest, stomach, back, thumbs, or toes
  • Acne, oily skin, or dandruff
  • Pelvic pain
  • Weight gain or obesity, usually carrying extra weight around the waist
  • Type 2 diabetes
  • High cholesterol
  • High blood pressure
  • Male-pattern baldness or thinning hair
  • Patches of thickened and dark brown or black skin on the neck, arms, breasts, or thighs
  • Skin tags, or tiny excess flaps of skin in the armpits or neck area

Investigations & tests to diagnose Polycystic Ovarian Syndrome (PCOS)
There is no single test to diagnose PCOS. Your doctor will take a medical history, perform a physical exam—possibly including an ultrasound, check your hormone levels, and measure glucose, or sugar levels, in the blood. If you are producing too many male hormones, the doctor will make sure it’s from PCOS. At the physical exam the doctor will want to evaluate the areas of increased hair growth, so try to allow the natural hair growth for a few days before the visit. During a pelvic exam, the ovaries may be enlarged or swollen by the increased number of small cysts. This can be seen more easily by vaginal ultrasound, or screening, to examine the ovaries for cysts and the endometrium. The endometrium is the lining of the uterus. The uterine lining may become thicker if there has not been a regular period. 


How is Polycystic Ovarian Syndrome (PCOS) treated?
Because there is no cure for PCOS, it needs to be managed to prevent problems. Treatments are based on the symptoms each patient is having and whether she wants to conceive or needs contraception. Below are descriptions of treatments used for PCOS.
  • Birth control pills. For women who don’t want to become pregnant, birth control pills can regulate menstrual cycles, reduce male hormone levels, and help to clear acne. However, the birth control pill does not cure PCOS. The menstrual cycle will become abnormal again if the pill is stopped. Women may also think about taking a pill that only has progesterone, like Provera, to regulate the menstrual cycle and prevent endometrial problems. But progesterone alone does not help reduce acne and hair growth.
  • Diabetes MedicationsThe medicine, Metformin, also called Glucophage, which is used to treat type 2 diabetes, also helps with PCOS symptoms. Metformin affects the way insulin regulates glucose and decreases the testosterone production. Abnormal hair growth will slow down and ovulation may return after a few months of use. These medications will not cause a person to become diabetic.
  • Fertility MedicationsThe main fertility problem for women with PCOS is the lack of ovulation. Even so, her husband’s sperm count should be checked and her tubes checked to make sure they are open before fertility medications are used. Clomiphene (clomid) medication and gonadotropin injections can be used to stimulate the ovary to ovulate. PCOS patients are at increased risk for multiple births when using these medications. In vitro Fertilization (IVF) is sometimes recommended tocontrol the chance of having triplets or more. Metformin can be taken with fertility medications and helps to make PCOS women ovulate on lower doses of medication.
  • Medicine for increased hair growth or extra male hormonesIf a woman is not trying to get pregnant there are some other medicines that may reduce hair growth. Spironolactone is a blood pressure medicine that has been shown to decrease the male hormone’s effect on hair. Propecia, a medicine taken by men for hair loss, is another medication that blocks this effect. Both of these medicines can affect the development of a male foetus and should not be taken if pregnancy is possible. Other non-medical treatments such as electrolysis or laser hair removal are effective at getting rid of hair. A woman with PCOS can also take hormonal treatment to keep new hair from growing.
  • Surgery. Although it is not recommended as the first course of treatment, surgery called ovarian drilling is available to induce ovulation. The doctor makes a very small incision above or below the navel, and inserts a small instrument that acts like a telescope into the abdomen. This is called laparoscopy. The doctor then punctures the ovary with a small needle carrying an electric current to destroy a small portion of the ovary. This procedure carries a risk of developing scar tissue on the ovary. This surgery can lower male hormone levels and help with ovulation. But these effects may only last a few months. This treatment doesn’t help with increased hair growth and loss of scalp hair.
  • A healthy weight. Maintaining a healthy weight is another way women can help manage PCOS. Since obesity is common with PCOS, a healthy diet and physical activity help maintain a healthy weight, which will help the body lower glucose levels, use insulin more efficiently, and may help restore a normal period. Even loss of 10% of her body weight can help make a woman’s cycle more regular.


How does Polycystic Ovarian Syndrome (PCOS) affect a woman while pregnant?
There appears to be a higher rate of miscarriage, gestational diabetes, pregnancy-induced high blood pressure, and premature delivery in women with PCOS. Researchers are studying how the medicine, metformin, prevents or reduces the chances of having these problems while pregnant, in addition to looking at how the drug lowers male hormone levels and limits weight gain in women who are obese when they get pregnant. 
Note: No one yet knows if metformin is safe for pregnant women. Because the drug crosses the placenta, doctors are concerned that the baby could be affected by the drug. Research is ongoing.


Does Polycystic Ovarian Syndrome (PCOS) put women at risk for other conditions?
Women with PCOS can be at an increased risk for developing several other conditions. Irregular menstrual periods and the absence of ovulation cause women to produce the hormone estrogen, but not the hormone progesterone. Without progesterone, which causes the endometrium to shed each month as a menstrual period, the endometrium becomes thick, which can cause heavy bleeding or irregular bleeding. Eventually, this can lead to endometrial hyperplasia or cancer. Women with PCOS are also at higher risk for diabetes, high cholesterol, high blood pressure, and heart disease. Getting the symptoms under control at an earlier age may help to reduce this risk.
Does Polycystic Ovarian Syndrome (PCOS) change at menopause?
Researchers are looking at how male hormone levels change as women with PCOS grow older. They think that as women reach menopause, ovarian function changes and the menstrual cycle may become more normal. But even with falling male hormone levels, excessive hair growth continues, and male pattern baldness or thinning hair gets worse after menopause.

OBESITY
Obesity in absolute terms is an increase of body fatty tissue mass. In a practical setting it is difficult to measure this directly, and obesity is typically measured by BMI (body mass index) and in terms of its distribution through waist circumference or waist-hip circumference ratio measurements. BMI
Body mass index or BMI is a simple and widely used method for estimating body fat mass. BMI was developed in the 19th century by the Belgian statistician and anthropometrist Adolphe Quetelet. BMI is an accurate reflection of body fat percentage in the majority of the adult
population, but is less accurate in situations that affect body composition such as in body builders and pregnancy
BMI is calculated by dividing the subject’s weight by the square of his/her height, typically expressed either in metric or US “Customary” units:
Metric: BMI = kg / m2
Where kg is the subject’s weight in kilograms and m is the subject’s height in metres.
US/Customary and imperial: BMI = lb * 703 / in
Where lb is the subject’s weight in pounds and in is the subject’s height in inches
The most commonly used definitions, established by the WHO in 1997 and published in 2000, provide the following values:
  • A BMI less than 18.5 is underweight
  • A BMI of 18.5–24.9 is normal weight
  • A BMI of 25.0–29.9 is overweight
  • A BMI of 30.0–34.9 is class I obesity
  • A BMI of 35.0-39.9 is class II obesity
  • A BMI of > 40.0 is class III obesity or severe / morbidly obese
  • A BMI of 35.0 or higher in the presence of at least one other significant comorbidity is also classified by some bodies as morbid obesity.


Causes
Most researchers agree that a combination of excessive calorie consumption and a sedentary lifestyle are the primary causes of obesity in the majority of the population. Other less well established or minor influences include genetic causes, medical and psychiatric illnesses, and microbiological causes. 
A  review identifies ten other possibly under investigated causes for recently increasing rates of obesity:
(1) Insufficient sleep
(2) Endocrine disruptors – food substances that interfere with lipid metabolism,
(3) decreased variability in ambient temperature,
(4) decreased rates of smoking, which suppresses appetite,
(5) increased use of medication that leads to weight gain,
(6) Increased distribution of ethnic and age groups that tend to be heavier,
(7) Pregnancy at a later age,
(8) Intrauterine and intergenerational effects,
(9) Positive natural selection of people with a higher BMI,
(10) Assortative mating, heavier people tending to form relationships with each other.


Dietary
Despite the widespread availability of nutritional information in schools, doctors’ offices, on the internet and on product packaging, it is evident that overeating remains a substantial problem.
Most of these extra calories came from an increase in carbohydrate consumption rather than an increase in fat consumption.


Sedentary lifestyle
An increasingly sedentary lifestyle plays a significant role in obesity. There has been a trend toward decreased physical activity due in part to increasingly mechanized forms of work, changing modes of transportation, and increasing urbanization. Studies in children and adults have found an association between the number of hours of television watched and the prevalence of obesity. Driving one’s children to school also decreases the amount of exercise that these children get and has led to calls for reduced car use around schools.


Genetics
Like many other medical conditions, obesity is the result of interplay between genetic and environmental factors. Polymorphisms in various genes controlling appetite, metabolism, and adipokine release may predispose to obesity when sufficient calories are present. Obesity is a major feature in a number of rare genetic conditions: Prader-Willi syndrome, Bardet-Biedl syndrome, MOMO syndrome, leptin receptor mutations, and melanocortin receptor mutations. In a people with early-onset severe obesity (defined by an onset before ten years of age and body mass index over three standard deviations above normal),
On a population level, the thrifty gene hypothesis postulates that certain ethnic groups may be more prone to obesity when exposed to an equivalent environment. Their ability to take advantage of rare periods of abundance by storing energy as fat would be advantageous during times of varying food availability. Individuals with greater adipose reserves would be more likely survive famine. This tendency to store fat however would be maladaptive in societies with stable food supplies.


Medical illness
Certain physical and mental illnesses and the pharmaceutical substances used to treat them can increase one’s risk of obesity. Medical illnesses that increase obesity risk include several rare genetic syndromes (listed above) as well as some congenital or acquired conditions: (1) hypothyroidism, (2) Cushing’s syndrome, (3) growth hormone deficiency, and (4) eating disorders such bulimia nervosa, binge eating disorder and compulsive overeating. Certain medications may cause weight gain and or negative changes in body composition, such as steroids, atypical antipsychotics, some fertility medication, insulin and sulfonylureas.


Microbiological
The role of bacteria colonizing the digestive tract in the development of obesity has recently become the subject of investigation. Bacteria participate in digestion (especially of fatty acids and polysaccharides), and alterations in the proportion of particular strains of bacteria may explain why certain people are more prone to weight gain than others. Human digestive tract bacteria are generally either member of the phyla of bacteroidetes or of firmicutes. In obese people, there is a relative abundance of firmicutes (which cause relatively high energy absorption), which is restored by weight loss. From these results it cannot be concluded whether this imbalance is the cause or effect of obesity.


Social determinants
The correlation between social class and BMI is inconsistent. Comparing net worth with BMI found obese Americanians  approximately half as wealthy as thin ones. When income differentials were factored out, the inequity persisted. A tendency to rely on fast food is seen as one of the reasons why this result occurred. Another study found women who married into a higher social class are thinner than women who married into a lower social class.

What is Masturbation?

sperm.)  Heavy drinking can affect a woman’s menstrual cycle as well. Note however, that as long as drinking, smoking and drug use have been curtailed before getting pregnant, prior use seems to pose almost no risk to the baby.
 
Ovulation/menstruation problems
Ovulation is a pre-requisite for getting pregnant.  In women with regular menstrual cycles of 28-35 days, ovulation takes place once every cycle.  However, many women have erratic menstrual cycles (often getting periods 3-4 times a year or even less).  This means that they have fewer chances of getting pregnant not only because they have fewer possible fertile
periods, but also because they may not be ovulating in every cycle.  (Note that ovulation does not necessarily take place in every menstrual cycle).
 
Low sperm count (for males)
The normal sperm count ranges from 20 million to 100 million sperm per cubic centimetre. Counts below 20 million may be a cause for difficulty in conception. Factors such as fatigue, excessive alcohol, smoking or other drug abuse, prostate gland infection, poor diet, or occupational exposure to chemicals can cause a small, temporary decrease in the sperm count. Varicose is a condition when there are varicose veins on the scrotum. This is another cause
for infertility and can only be corrected through surgery.
 
Retroverted uterus
Many women are born with their uteruses facing backward (retroverted) instead of forward.  This condition can be diagnosed either by your gynaecologist, or through an ultrasound.  One simple way to compensate for this problem of a retroverted uterus is for the woman to lie on her stomach after intercourse.  (Women with regular uteruses who are planning to get
pregnant are asked to lie on their backs and lift their legs from their hips after sex in order to improve the chances of fertilization).  If retroverted uterus is the only problem, then in most cases the woman will conceive.  However, if she still does not get pregnant, then the doctor
may advise surgery.
 
Polycystic ovaries
Cysts are formed when the follicle fails to rupture at the time of ovulation and begins to grow instead. This condition occurs due to hormonal imbalance and is marked by absent or infrequent menstruation. Women having this condition have a chronic tendency to have their periods at intervals ranging from every six weeks to six months. Conception becomes difficult due to
irregular ovulation. 
 
Intercourse pattern
Very often, you may simply be mistiming your intercourse.  In order for you to get pregnant, it is essential for you to have sex during your fertile days. It could also be possible that you may be trying too often in a particular cycle, and thereby causing a reduction in sperm count.

All of us go through a brief interlude of doubt and concern when we do not achieve pregnancy the very first month we try – and we start wondering about our fertility. 
 

Only a 25% chance!

Before worrying, remember that in a single menstrual cycle, the chance of a perfectly normal couple achieving a successful pregnancy is only about 25%, even if they have sex every single day. This is called their fecundity, which describes their fertility potential. Humans are not very efficient at producing babies! There are many reasons for this, including the fact that some eggs don’t fertilize and some of the fertilized eggs don’t grow well in the early developmental stage.
Getting pregnant is a game of odds – it’s a bit like playing Russian Roulette and it’s impossible to predict when an individual couple will get pregnant! However, over a period of a year, the  chance of a successful pregnancy is between 80 and 90%; so that 7 out of 8 couples will be pregnant within a year. These are the normal “fertile” couples – and the rest are “labelled” infertile – the medical text book definition of infertility being the inability to conceive even after trying for a year. Couples, who have never had a child, are said to have “primary infertility,” those who have become pregnant at least once but are unable to conceive again, are said to have “secondary infertility.”
 
Factors that determine fertility
The chances of pregnancy for a couple in a given cycle will depend upon many things, and the most important of these are:
·        The age of the woman. At the biologic clock ticks on, the number of eggs and their quality starts decreasing.
·       Frequency of intercourse. While there is no “normal” frequency for sex, the “optimal” frequency of intercourse if you are trying to get pregnant is about 3 times a week in the fertile period. Simply stated, the more sex the better! Couples, who have  intercourse less frequently, have a diminished chance of conceiving.
·       “Trying time” – that is, how long the couple have been trying to get pregnant. This is an important concept. The longer a couple has been trying to conceive without success, the lesser their chances of getting pregnant without medical help.
·        The presence of fertility problems.
 
What happens when a couple has a fertility problem?
The chances of their getting pregnant depend upon a number of variables multiplied together. Consider a couple where both the husband and wife have a condition that impairs their fertility. For example, the husband’s fertility, based on a reduced sperm count is 50 percent of normal values. His wife ovulates only in 50 percent of cycles; and one of her fallopian tubes is blocked. With three relative infertility factors, their chance of conception is 0.5 (sperm count) X 0.5 (ovulation factor) X 0.5 (Tubal factor) = 0.125, or 12.5 percent of normal. Since the chance of conception in normal fertile couples is only 25% in any one cycle, the probability of pregnancy in any given month for this couple without treatment is only 3 percent (0.125 X 25 = 0.03125)! Even if they kept on trying for 5 years, their chance of conceiving on their own would be 60% only. Thus, infertility problems multiply together and magnify the odds against a couple achieving a pregnancy. 
This is why it is important to correct or improve each partner’s contributing infertility factors as much as possible in order to maximize the chances of conception.
If infertile couples had 300 years in which to breed, most wives would get pregnant without any treatment at all! Of course, time is at a premium, so the odds need to be improved – and this is
where medical treatment comes in.
 
Causes of Male Infertility 
Indications for male infertility
Statistics show that in at least 40% of infertile couples, there is a male factor contributing to the problem. Male fertility screening is done through semen analysis. The general rule is that the lower the sperm count and the poorer the sperm quality, the longer it will take and the more difficult it may be for a pregnancy to occur. However, even men with very low sperm counts may eventually be successful in causing a pregnancy. The sperm count only needs to be high enough for that one time that ovulation occurs, in order for a pregnancy to be possible. 
 
What are the major factors affecting sperm production
Oligospermia is the term used to describe the condition when the sperm count is low. If there are no sperm at all in the semen sample, the condition is termed azoospermia. In such cases,
the clinic will wish to see your husband to take a more detailed history and to examine him.  In his history, specific questions will be asked about any past condition that may have affected the testicles. For instance, he will have to answer queries regarding whether he has had any operations in the groin area or for undescended testicles in childhood. The specialist will also ask
questions to determine whether there has been any major injury or infection in the genital area.
Infected semen can be a cause of infertility. Earlier, it was believed that mumps could cause sterility in men.
However, it is now known that even when the testicles are involved in mumps orchitis (very painful swelling of the testicles); it is extremely rare for this to lead to sterility. 
 
What other factors affect sperm production
  1. Heat can have a detrimental effect on normal sperm production. It is for this reason that nature has placed the testicles outside the body in the scrotum rather than in the abdomen like the ovaries. Some infertile men soak themselves in a bathtub full of scalding water and emerge looking like lobsters. This can almost stop sperm production completely. Obese men can also become sterile because the sagging layers of fat can overheat the testicles. Men whose jobs involve long hours of sitting, e.g. long distance lorry-drivers may have infertility due to the increased heat to the genital area.
  2. Very frequent intercourse can lead to the demand exceeding the supply. There are quite a number of infertile men whose sex drive is such that they must ejaculate 2-3 times a day. This explodes the myth that links infertility to lack of virility. It can be extremely difficult for these men to reduce their ejaculation rate to every 48 hours.
  3. Smoking over 20 cigarettes a day has been shown to reduce both the sperm count and
    the sperm motility to quite a major degree.
  4. Excessive alcohol intake will lead to infertility mainly because a man loses both the inclination and the ability to rise to the occasion! Alcohol can also lower the production of sperm and of the male hormone testosterone.
  5. The workaholic husband can find that fatigue can have similar effects on his interest in intercourse as excessive alcohol intake.
 
What problems can be identified during examination
Every infertile man must be carefully examined. The examination of the external genitalia in the male rarely shows anything out of the ordinary. However, sometimes one or both testicles are very small or rarely may even be absent. Additionally there may be congenital absence of each vas deferens, the ducts through which sperm pass from the testicles to the female at intercourse.
Other factors which may contribute to lowered fertility, and which can be identified on examination include:
  1. Varicocele is a term used to describe a condition where there are varicose veins around the testicle and vas deferens. In the case of varicoceles, there is an increase in the blood flow and temperature around the testicle. Even a small varicocele may be significant and affect sperm production.
  2. The presence of excessive fluid around the testicle is known as a hydrocele.
  3. There can occasionally be anatomical defects in the development of the penis.  Consequently, during intercourse and ejaculation the sperm cannot be deposited within the vagina very easily. Retrograde ejaculation is a rare condition when some men ejaculate backwards into the bladder. This could be the result of earlier surgery to the urethra (the outflow tube from the bladder).
  4. A rectal examination can help determine whether the prostate gland may be a source
    of chronic infection.
 
Various Male Fertility Disorders
Azoospermia
Azoospermia characterized by the absence of sperm is either due to an obstruction in the outflow system from the testicle, in the epididymis or vas deferens, or due to a failure in sperm production (spermatogenesis). If a biopsy of the testicle showed that spermatogenesis was normal, this would then indicate that an obstruction was the cause of his azoospermia. Vasograms, which are X-rays of the vas deferens can then be performed to identify the location
of the obstruction.
 
Presence of Antibodies
Sometimes, a semen sample when seen under a microscope, will show sperm clumping. This may indicate the presence of sperm antibodies that are causing the sperm to stick together. A blood sample can be tested for the presence of agglutinating and other sperm inhibiting antibodies.
 

Klinefelter’s  Syndrome

Very small testicles may be the manifestation of Klinefelter’s Syndrome. This is caused by a chromosomal aberration. Thus, a chromosome investigation will show the presence of an extra X (female) chromosome.
 
Hormone Disorders
Hormone disorders are rare causes of male infertility, but it may sometimes be helpful to check his F.S.H., L.H., testosterone (male hormone), prolactin and thyroid hormone levels.
 
Bacterial Infection
The existence of an excessive amount of white blood cells in the semen sample may indicate that an infection may be reducing the ability of the sperm to fertilise an egg. The semen sample should then be cultured to determine the type of infecting bacterial organism.

Irritable bowel syndrome (IBS) is a “syndrome,” meaning a group of symptoms. Doctors are not sure what causes IBS. The nerves and muscles in the bowel appear to be extra sensitive in people with IBS. Muscles may contract too much when you eat. IBS can be painful.
But it does not damage the colon or other parts of the digestive system. IBS does not lead to other health problems.

Cirrhosis of the Liver
The liver, the largest organ in the body, is essential in keeping the body functioning properly. It removes or neutralizes poisons from the blood, produces immune agents to control infection, and removes germs and bacteria from the blood. It makes proteins that regulate blood clotting and produces bile to help absorb fats and fat-soluble vitamins. You cannot live without a functioning liver.


In cirrhosis of the liver, scar tissue replaces normal, healthy tissue, blocking the flow of blood through the organ and preventing it from working as it should. Cirrhosis is the twelfth leading cause of death by disease, killing about 26,000 people each year. Also, the cost of cirrhosis in terms of human suffering, hospital costs, and lost productivity is high.


Causes
Cirrhosis has many causes. In the United States, chronic alcoholism and hepatitis C are the most common ones.


Alcoholic liver disease. Too many people, cirrhosis of the liver is synonymous with chronic alcoholism, but in fact, alcoholism is only one of the causes. Alcoholic cirrhosis usually develops after more than a decade of heavy drinking. The amount of alcohol that can injure the liver varies greatly from person to person. In women, as few as two to three drinks per day have been linked with cirrhosis and in men, as few as three to four drinks per day. Alcohol seems to injure the liver by blocking the normal metabolism of protein, fats, and carbohydrates.


Chronic hepatitis C. The hepatitis C virus ranks with alcohol as a major cause of chronic liver disease and cirrhosis in the United States. Infection with this virus causes inflammation of and low grade damage to the liver that over several decades can lead to cirrhosis.


Chronic hepatitis B and D. The hepatitis B virus is probably the most common cause of cirrhosis worldwide, but it is less common in the United States and the Western world. Hepatitis B, like hepatitis C, causes liver inflammation and injury that over several decades can lead to cirrhosis. Hepatitis D is another virus that infects the liver, but only in people who already have hepatitis B.


Autoimmune hepatitis. This disease appears to be caused by the immune system attacking the liver and causing inflammation, damage, and eventually scarring and cirrhosis.


Inherited diseases. Alpha-1 antitrypsin deficiency, hemochromatosis, Wilson disease, galactosemia, and glycogen storage diseases are among the inherited diseases that interfere with the way the liver produces, processes, and stores enzymes, proteins, metals, and other substances the body needs to function properly.


Non-alcoholic steatohepatitis (NASH). In NASH, fat builds up in the liver and eventually causes scar tissue. This type of hepatitis appears to be associated with diabetes, protein malnutrition, obesity, coronary artery disease, and treatment with corticosteroid medications.


Blocked bile ducts. When the ducts that carry bile out of the liver are blocked, bile backs up and damages liver tissue. In babies, blocked bile ducts are most commonly caused by biliary atresia, a disease in which the bile ducts are absent or injured. In adults, the most common cause is primary Biliary cirrhosis, a disease in which the ducts become inflamed, blocked, and scarred. Secondary Biliary cirrhosis can happen after gallbladder surgery if the ducts are inadvertently tied off or injured.


Drugs, toxins, and infections. Severe reactions to prescription drugs, prolonged exposure to environmental toxins, the parasitic infection schistosomiasis, and repeated bouts of heart failure
with liver congestion can all lead to cirrhosis. 


Symptoms
Many people with cirrhosis have no symptoms in the early stages of the disease. However, as scar tissue replaces healthy cells, liver function starts to fail and a person may experience the following symptoms:
·        Exhaustion
·        Fatigue
·        Loss of appetite
·        Nausea
·        Weakness
·        Weight loss
·        Abdominal pain
·        Spider-like blood vessels (spider angiomas) that develop on the skin
As the disease progresses, complications may develop. In some people, these may be the first signs of the disease.

Causes and Symptoms of hair loss
1-Pattern baldness (androgenetic alopecia).
In male- and female-pattern baldness, the time of growth shortens, and the hairs are not as thick or sturdy. With each growth cycle, the hairs become rooted more superficially and more easily fall out. Heredity likely plays a key role. A history of androgenetic alopecia on either side of
your family increases your risk of balding. Heredity also affects the age at which you begin to lose hair and the developmental speed, pattern and extent of your baldness.
 
Symptoms
Male-pattern baldness- MPB-(androgenetic alopecia)...For men, pattern baldness can begin very early, even in the teens or early 20s. It’s typically characterized by a receding hairline at the temples and balding at the top of the head. The end result may be partial or complete baldness
Female-pattern baldness-FPB- (androgenetic alopecia). Women with permanent hair loss usually have hair loss limited to thinning at the front, sides or crown. Women usually maintain their frontal hairline and rarely experience complete baldness.
 
2-Cicatricial (scarring) alopecia.
This type of permanent hair loss occurs when inflammation damages and scars the hair follicle. This prevents new hair from growing. This condition can be seen in several skin conditions, including lupus erythematosus or lichen planus. It’s not known what triggers or causes this inflammation.
 
Symptoms: This rare condition occurs when inflammation damages and scars the hair follicle, causing permanent hair loss. Sometimes the patchy hair loss is associated with slight itching or pain.
 
3-Alopecia areata.
This is classified as an autoimmune disease, but the cause is unknown. People who develop alopecia areata are generally in good health. A few people may have other autoimmune disorders including thyroid disease. Some scientists believe that some people are genetically predisposed to develop alopecia areata and that a trigger, such as a virus or something else in the environment, sets off the condition. A family history of alopecia areata makes you more likely to develop it. With alopecia areata, your hair generally grows back, but you may lose and regrow your hair a number of times.
 
Symptoms: Hair loss usually occurs in small, round, smooth patches about the size of a quarter. Usually the disease doesn’t extend beyond a few bare patches on the scalp, but it can cause patchy hair loss on any area that has hair, including eyebrows, eyelashes and beard. In rare cases, it can progress to cause hair loss over the entire body. If the hair loss includes your entire scalp, the condition is called alopecia totalis. If it involves your whole body, it’s called alopecia universalis. Soreness and itching may precede the hair loss.
 
4-Telogen effluvium.
This type of hair loss is usually due to a change in your normal hair cycle. It may occur when some type of shock to your system — emotional or physical — causes hair roots to be pushed prematurely into the resting state. The affected growing hairs from these hair roots fall out. In a month or two, the hair follicles become active again and new hair starts to grow. Telogen
effluvium may follow emotional distress, such as a death in the family, or after a physiological stress, such as a high fever, sudden or excessive weight loss, nutritional deficiencies, surgery, or metabolic disturbances. Hair typically grows back once the condition that caused it corrects itself, but it usually take months.
 
Symptoms: This type of temporary hair loss occurs suddenly. Handfuls of hair may come out when combing or washing your hair or may fall out after gentle tugging. This type of hair loss usually causes overall hair thinning and not bald patches.
 
5-Traction alopecia.
Excessive hairstyling or hairstyles that pull your hair too tightly cause traction alopecia. If the pulling is stopped before there’s scarring of your scalp and permanent damage to the root, hair usually grows back normally.
 
Symptoms: Bald patches can occur if you regularly wear certain hairstyles, such as pigtails, braids or cornrows, or if you use tight rollers. Hair loss typically occurs between the rows or at the part where hair is pulled tightly.
 
Other causes of hair loss
  • Poor nutrition. Having inadequate protein or iron in your diet or poor nourishment in other ways can cause you to experience hair loss. Fad diets, crash diets and certain illnesses, such as eating disorders, can cause poor nutrition.
  • Medications. Certain drugs used to treat gout, arthritis, depression, heart problems and high blood pressure may cause hair loss in some people. Taking birth control pills also may result in hair loss for some women.
  • Disease. Diabetes and lupus can cause hair loss.
  • Medical treatments. Undergoing chemotherapy or radiation therapy may cause you to develop alopecia. Under these conditions, healthy, growing (anagen) hairs can be affected. After your treatment ends, your hair typically begins to regrow.
  • Hormonal changes. Hormonal changes and imbalances can cause temporary hair loss. This could be due to pregnancy, having a baby, discontinuing birth control pills, beginning menopause, or an overactive or under active thyroid gland. The hair loss may be delayed by three months following a hormonal change, and it’ll take another three months for new hair to grow back. During pregnancy, it’s normal to have thicker, more luxuriant hair. It’s also common to lose more hair than normal about three months after delivery. If a hormonal imbalance is associated with an overproduction of testosterone, there may be a thinning of hair over the crown of the scalp. Correcting hormonal imbalances may stop hair loss.
  • Hair treatments. Chemicals used for dying, tinting, bleaching, straightening or permanent waves can cause hair to become damaged and break off if they are overused or used incorrectly. Over styling and excessive brushing also can cause hair to fall out if the hair
    shaft becomes damaged. 
  • Scalp infection. Infections, such as ringworm, can invade the hair and skin of your scalp, leading to hair loss. Once infections are treated, hair generally grows back. Ringworm, a fungal infection, can usually be treated with a topical or oral antifungal medication.
  • Trichotillomania (hair-pulling disorder). Trichotillomania is a type of mental illness in which people have an irresistible urge to pull out their hair, whether it’s from their scalp, their eyebrows or other areas of their body. Hair pulling from the scalp often leaves them with patchy bald spots on their head, which they may go to great lengths to disguise. Causes of trichotillomania are still being researched, and no specific cause has yet been found.

Gallstones – Gall Bladder Stones
The gallbladder contracts and pushes the bile into a tube—called the common bile duct—that carries it to the small intestine, where it helps with digestion.
Bile contains water, cholesterol, fats, bile salts, proteins, and bilirubin—a waste product. Bile salts break up fat, and bilirubin gives bile and stool a yellowish-brown color. If the liquid bile contains too much cholesterol, bile salts, or bilirubin, it can harden into gallstones.


Types of gallstones
1-Cholesterol stones
2- Pigment stones.
Cholesterol stones are usually yellow-green and are made primarily of hardened cholesterol. Pigment stones are small, dark stones made of bilirubin.
Gallstones can be as small as a grain of sand or as large as a golf ball. The gallbladder can develop just one large stone, hundreds of tiny stones, or a combination of the two.
Gallstones can block the normal flow of bile if they move from the gallbladder and lodge in any of the ducts that carry bile from the liver to the small intestine.
If any of the bile ducts remain blocked for a significant period of time, severe damage or infection can occur in the gallblad­der, liver, or pancreas. Left untreated, the condition can be fatal. Warning signs of a serious problem are fever, jaundice, and persistent pain.


Causes for gallstones
Scientists believe cholesterol stones form when bile contains too much cholesterol, too much bilirubin, or not enough
bile salts, or when the gallbladder does not empty completely or often enough.
The reason these imbalances occur is not known.
The cause of pigment stones is not fully understood. The stones tend to develop in people who have liver cirrhosis, Biliary tract infections, or hereditary blood disorders—such as sickle cell anaemia—in which the liver makes too much bilirubin.


The mere presence of gallstones may cause more gallstones to develop. Other factors that contribute to the formation of gallstones, particularly cholesterol stones, include

  • Sex. Women are twice as likely as men to develop gallstones. Excess oestrogen from pregnancy, hormone replace­ment therapy, and birth control pills appears to increase cholesterol levels in bile and decrease gallbladder move­ment, which can lead to gallstones.
  • Family history. Gallstones often run in families, pointing to a possible genetic link.
  • Weight. A large clinical study showed that being even moderately over­weight increases the risk for developing gallstones. The most likely reason is that the amount of bile salts in bile is reduced, resulting in more choles­terol. Increased cholesterol reduces gallbladder emptying. Obesity is a major risk factor for gallstones, especially in women.
  • Diet. Diets high in fat and cholesterol and low in fiber increase the risk of gallstones due to increased cholesterol in the bile and reduced gallbladder emptying.
  • Rapid weight loss. As the body metabolizes fat during prolonged fasting and rapid weight loss—such as “crash diets”—the liver secretes extra cholesterol into bile, which can cause gallstones. In addition, the gallblad­der does not empty properly.
  • Age. People older than age 60 are more likely to develop gallstones than younger people. As people age, the body tends
    to secrete more cholesterol into bile.
  • Ethnicity. American Indians have a genetic predisposition to secrete high levels of cholesterol in bile. In fact, they have the highest rate of gallstones in the United States. The majority of American Indian men have gallstones by age 60. Among the Pima Indians of Arizona, 70 percent of women have gallstones by age 30. Mexican American men and women of all ages also have high rates of gallstones.
  • Cholesterol-lowering drugs. Drugs that lower cholesterol levels in the blood actually increase the amount of cholesterol secreted into bile. In turn, the risk of gallstones increases.
  • Diabetes. People with diabetes gener­ally have high levels of fatty acids called triglycerides. These fatty acids may increase the risk of gallstones. 


Symptoms of gallstones
  • Steady pain in the right upper abdo­men that increases rapidly and lasts from 30 minutes to several hours
  • Pain in the back between the shoulder blades
  • Pain under the right shoulder. Notify your doctor if you think you have experienced a gallbladder attack. Although these attacks often pass as gallstones move, your gallbladder can become infected and rupture if a blockage remains.
  • People with any of the following symptoms should see a doctor immediately:
  • prolonged pain—more than 5 hours
  • Nausea and vomiting.
  • Fever—even low-grade—or chills
  • Yellowish colour of the skin or whites of the eyes
  • Clay-colored stools
Many people with gallstones have no symp­toms; these gallstones are called “silent stones.” They do not interfere with gall­bladder, liver, or pancreas function and do not need treatment.


Non surgical Treatment
Non surgical approaches are used only in special situations—such as when a patient has a serious medical condition needs surgery—and only for cholesterol stones. Stones commonly recur within 5 years in patients treated non surgically.

Eosinophilia
Eosinophils are a type of white blood cell. They contain particles filled with chemicals that fight off infections and play a role in your body’s immune response. Normally your blood doesn’t have a large number of Eosinophils. Your body may produce more of them in response to allergic disorders, inflammation of the skin, and parasitic infections. They can also increase in response to some infections or to some bone marrow disorders. In some conditions, the Eosinophils can move outside the bloodstream and into organs and tissues. 
Eosinophils usually account for less than 7% of the circulating leukocytes (100 to 500 Eosinophils per microliter of blood). These cells have a role in the protective immunity against certain parasites but also contribute to the inflammation that occurs in allergic disorders.
An increased number of Eosinophils in the blood (Eosinophilia) usually indicates the response of the body to abnormal cells, parasites, or substances that cause an allergic reaction (allergens).
Idiopathic hypereosinophilic syndrome is a disorder in which the number of Eosinophils increases to more than 1,500 cells per microliter of blood for more than 6 months without an obvious cause.
People of any age can develop idiopathic hypereosinophilic syndrome, but it is more common in men older than 50. The increased number of Eosinophils can damage the heart, lungs, liver, skin, and nervous system. For example, the heart can become inflamed in  a condition called Löffler’s endocarditis, leading to formation of blood clots, heart failure, heart attacks, or malfunctioning heart valves.
Signs and Symptoms
